School Based Health Services Agreement Application Instructions


1. Through your Single Sign-On, go to “GMS Access / Select” and scroll to available applications under IDEA.

2. Click “Create” on the line for “School Based Services Participation Agreement.

    Once you have created the application it will appear under your applications.

3. Please agree to all assurances by clicking on each tab.

4. Under the signature tab, upload the full contract, including the signature page. Then submit.
