This request MUST BE PRE-APPROVED by a cabinet member/executive director. By submitting the approvers name, YOU ARE AGREEING you have discussed this project in detail with cabinet member/executive director and content is finalized.
Project Information
Please be aware there is a MINIMUM OF TWO WEEKS IN ADVANCE before the deadline. Please plan accordingly.
Please be aware there is a 2-week turnaround in place.
Please describe all uses and audiences for this document: OSDE website, legislature, conference attendees, etc.
The same design may be used for multiple items and may be filled out on the same request. Different setups require separate requests.
Please provide as much information as possible. Failure to provide a clear and concise direction for this request may result in a longer turn around time for your project.
