Special Education Early Childhood Provider Resources

The following links serve as provider resources for individuals caring for children ages 3 through 5.


Child Outcome Summary Forms (COSF)

Child Outcome Summary Forms are used by providers serving children 0-3 years of age in Sooner Start Early Intervention, and providers serving children 3-5 years of age in local education agencies. These forms must be completed twice for children who have received services for 6 months or more. Once at the child's entry into the program and again at the child’s entrance into kindergarten. Best practice is to use two or more sources of information when completing these forms. Keep in mind; these forms and the numerical scale are based on typical child development.

The OSDE-COSF forms:

  • Track progress of children 0-6 years old who have received services for at least six months.  
  • Provide data on the effectiveness of early intervention and early childhood programs.  
  • Forms are based on typical child development on a 1-7 numerical scale. 
  • Entry and exit forms must be completed if a child receives services for 6 months or more.

Early Childhood Program Codes

Parents as Teachers provides information on the home visiting program available to families birth to age three. Thirty-seven Oklahoma education agencies have received grants to provide services for the 2013-2014 school year.

The Council for Exceptional Children, Division of Early Childhood provides standards for educating young children. 

Professional Development Materials and Training provides presentations and resources on early childhood outcomes and early childhood program codes. 

Child Outcomes Summary-Team Collaboration (COS-TC) Quality Practice Checklist and Descriptions provides opportunities to practice with the checklist and descriptions by watching video clips of COS team meetings with families and rating the extent to which providers in the video used COS-TC quality practices.
